Please print, review, sign, and bring the Informed Consent Agreement and completed Adult Information Form below to your first appointment, it will give us more time to talk during the initial assessment. If you do not have a printer, you will be provided with copies. Please arrive 20 minutes early to your scheduled appointment, in order to complete necessary paperwork. All documents are confidential.

FEES & PAYMENT:
Fees will be discussed and agreed upon during initial consultation/assessment. You may pay in cash or by check made out to Clymene Aldinger. A sliding scale fee is offered to students, dancers, freelance artists/creative professionals, and people experiencing economic hardship. Payment is due at the time each therapy session is held.
APPOINTMENTS:
Please provide 24-hour notice for all cancelled appointments. There is no penalty for canceling and rescheduling an appointment; however, you will be responsible for paying the full session fee for missed appointments.
